



Every season has a deadline. Tryouts do not wait, rosters fill up, and the coach is not going to hold a spot because the physical is not done yet. A sports physical, does not replace your child's annual physical, though it can be done during the annual physical if requested. It does not cover vaccines, behavioral health, development, or growth trends. What it does cover is everything that matters before your child puts on a uniform: heart function, joint stability, injury history, concussion status, breathing under exertion, and anything in their medical background that could become dangerous the moment practice begins.
A lot of parents assume the sports physical and the annual physical are interchangeable. They are not. At Burlington Pediatrics, we recommend combining them into a single appointment whenever possible. Your child gets cleared for the season and gets a complete well-child visit at the same time, covered under one insurance visit.
Sudden cardiac arrest is the leading cause of death in young athletes during exercise. It is rare, but it is also almost always preceded by warning signs that were never caught. During every sports physical in Burlington, we ask specifically about chest pain during exercise, unexplained fainting, dizziness on the field, and any family history of sudden cardiac death before age 50. We listen carefully. If anything raises concern, we do not guess we refer to a pediatric cardiologist before we sign anything.
A prior ankle sprain that never fully healed. A shoulder that pops. A knee that gives way occasionally. These are the things athletes learn to play through, and the things that turn into season-ending injuries if nobody catches them. We assess every major joint, evaluate the range of motion, and ask your athlete directly about pain they may not have mentioned at home. Student athletes are not always forthcoming with parents about what hurts. They are usually more forthcoming with us.
This is the part of the sports physical that gets the least attention and deserves the most. Any history of concussion, diagnosed or not, is reviewed carefully. If your child took a hit last season and felt foggy for a few days but never saw a doctor, we want to know. An unresolved concussion is one of the most common reasons for delayed clearance, and the only way to catch it is to ask the right questions.
Exercise-induced bronchoconstriction is extremely common in young athletes and goes undiagnosed. A child who gets winded faster than their teammates, coughs during sprints, or feels tightness in their chest during intense activity may have exercise-induced asthma, not poor conditioning. We screen for it specifically and can have a management plan in place before the first practice.
For athletes in wrestling, gymnastics, rowing, dance, and figure skating, we pay close attention to weight history, eating patterns, and any signs of disordered eating or unhealthy weight management. These sports carry a real risk of athletes pursuing dangerous weight behaviors that parents and coaches may not see. We create a confidential space for athletes to be honest.
Wrestlers, football players, and martial arts athletes are subject to MIAA rules around skin conditions. Active ringworm, impetigo, or herpes gladiatorum can result in disqualification from competition. We check, document, and if needed, treat so your athlete is not blindsided at weigh-ins or the night before a match.
Burlington High School runs 16 varsity programs across three seasons. Every athlete needs a current MIAA Pre-Participation Physical Evaluation form on file before the first practice of each season. We complete the official MIAA form at every applicable appointment. Bring it with you or download it from the MIAA website ahead of time.
The deadlines that catch families off guard every year:
Fall sports: football, soccer, field hockey, cross country, volleyball. Physical must be done before early August pre-season practice. Do not wait until the week before.
Winter sports: basketball, wrestling, swimming, hockey, and indoor track. Clear by early November before the November tryout window.
Spring sports: baseball, softball, lacrosse, tennis, golf, outdoor track. The most commonly missed deadline. Schedule in February, not April.

Youth leagues, travel organizations, and AAU programs each have their own physical requirements. Some accept a copy of the MIAA form. Some have their own paperwork. We handle whatever documentation your league requires and keep a copy in your child's file so you are never scrambling for records mid-season.


If your child's annual physical was done within the past year and was comprehensive, it will satisfy most sports camp requirements. We confirm this at the appointment and provide documentation in the format the camp needs.
Most athletes are cleared fully at their appointment. But sometimes we find something that needs to be addressed first, and that is exactly the point of the exam.
Common reasons we refer before clearing include an abnormal heart finding, a recent concussion that has not fully resolved, a joint injury that needs imaging, uncontrolled blood pressure, or a respiratory issue that needs a management plan before high-intensity activity is safe.
A referral is not a rejection. In nearly every case, the issue is resolved within a week or two, and your athlete is cleared and back on track. The alternative, missing something and having it become a medical emergency at practice, is not something we are willing to risk.

13 months from the date of the exam. A single comprehensive physical covers all sports your child plays within that year, fall, winter, and spring.
Possibly not. If the annual physical was comprehensive and recent, it may satisfy the sports physical requirement. Bring documentation of the annual physical to the appointment, and we will confirm on the spot whether additional evaluation is needed.
When combined with a comprehensive annual physical, most major plans, including BCBS, Tufts, and Harvard Pilgrim, cover the visit in full. A standalone sports-only physical may be billed differently depending on your plan. Call our front desk before your appointment, and we will give you an answer before you come in.
That depends entirely on their current status. If they completed a full Return to Play protocol, are symptom-free, and have been cleared by a physician, the answer is likely yes. If symptoms are ongoing or the concussion was recent, we will refer to a specialist before signing off. We do not clear concussions lightly.
